Animal Surrender Application

If you are interested in surrendering a pet, please fill out this form, and we will get back to you as soon as possible.  
 
We accept fish, invertebrates, reptiles, pet birds, exotic small mammals, and plants.
 
Domestic pets, such as dogs, cats, rabbits, guinea pigs, rats,  pigs, and chickens are not accepted. Owners of domestic animals should inquire with local pet shelters or rescues for options on rehoming their pets.

Wisconsin native animals that were taken from the wild are not accepted.  Owners of these animals should consult with a licensed wildlife rehabilitator for options.

Birds, reptiles, and mammals originating from outside WI must be accompanied by a Certificate of Veterinary Inspection (health paper) and a Import Permit through DATCP.  See: https://datcp.wi.gov/pages/programs_services/animalmovementpermits.aspx
